Montrose offers appointments to those requiring Functional Assessments. This is a great opportunity to get organised for potential NDIS applications you will need to make next year to achieve your goals. Having an in-depth Functional Assessment completed by an experienced Montrose Occupational Therapist helps to demonstrate what supports you need to achieve your care needs and goals.
During an assessment, we will collaborate with you to identify solutions and supports that will be most suitable for you. Your Occupational Therapist will work with you to identify your goals and how this might impact your day-to-day routines.
This includes;
Self-care
The Occupational Therapist will observe and discuss your completion of daily self-care such as overnight care needs, bathing, toileting, personal hygiene and dressing to identify any challenges.
Home
The Occupational Therapist will assess your living situation and current supports, and any difficulties you face while completing daily living tasks such as laundry, cleaning, grocery shopping and meal preparation.
Community
Having regular and easy access to your local community is essential for health and wellbeing. Your OT will assess your ability to get out and about in your local community to do things like banking, shopping and attending appointments and services.
Work and study
Your Occupational Therapist will discuss your current work and study routines, and goals you have. You will work together to identify current and potential challenges you may experience so that strategies can be implemented to support your participation in this area.
Leisure and social engagement
You may already be engaged in hobbies or recreational activities, or you may be looking to build your confidence and find new activities to participate in. Either way, your Occupational Therapist will work with you to identify challenges and establish strategies or goals to help you build a social and recreational lifestyle that suits you.
If you are looking to move to (SIL) Supported Independent Living accommodation, the NDIS will require a Functional Assessment to help determine what the ideal care package looks like for you and what accommodation would be most suitable to meet your needs.
